🦷 Dental Sealant Program – Consent Forms Going Home Today!
PCSD has partnered with the Florida Department of Health in Putnam County to offer a School-Based Dental Sealant Program for eligible students in 1st, 2nd, 5th and 6th grades!
The program provides dental sealants and other preventive dental services right at school — at no cost to families!
What parents need to know:
Check your child's backpack for a consent packet coming home today, November 2.
Complete all sections and sign the consent form.
Return the completed form to your child's teacher by November 20, 2026.
Participation requires a completed parent/guardian consent form.
Help us keep our students smiling and healthy! 💚💙
